Most Common Cause of Penalty ?

It depends if this relates to penalty shots imposed as a result of following the rules, e.g. for dropping out of a water hazard, or if it relates to penalties for breaching the rules. e.g. failing to drop the ball in the correct way.

If the former I'd say getting relief from a water hazard. If the latter, those are much rarer, but I'll go for touching the ground in a hazard.
 
improving your lie in a hazard,had to call a playing partner on sat for removing twigs from around his ball in a yellow staked hazard his comment bemused me " but its not in the water and we always do it!!!" this chap is in his 50's and has played for years,after he had his strop and i penalised his score he never spoke to me for the rest of the round.
 
A referee told me that if you see someone breaking a rule and you do not call him YOU are disqualified. Obviously we could claim ignorance of the rule but the referee explained to me that it would be difficult for him to claim ignorance of the rule!
 
Changing type of ball played.

Most common rule broken by club players IMO is assuming that a ball is lost in a hazard followed by improper drops.
when you say changing type of ball this would only be in a one ball only comp and not in general comps. only ever played in a one ball only and these were supplied by the sponsors.

It was not on the pro tour, it was club level golf. Cannot imagine it ever happenning in a pro event.

Not marking the ball when moving it for identification?

Groundng club in a hazard?

Not calling a provisional before playing it?

Holing out with the flag in?
