Ok, without looking anything up, I'd say playing 3 and he replaces ball on tee.
Shot 1 the air shot. Player "swings violently at the ball" so intention is to hit it so its a stroke and the ball is in play.
Shot 2 a penalty shot for hitting ball while swinging club but without intending to. Same as for hitting ball with practice swing. Stroke is cancelled ball is replaced - penalty one shot. So
Playing shot 3 from the tee. If original ball cannot be retrieved he can substitute another ball.
Incidently why do people think the fact that an OOB is behind a tee makes it irrelevant? The location of the OOB doesn't matter surely? OOB is OOB isn't it? If you hit a tee marker with your drive and the ball bounces behind you and goes OOB behind the tee its OOB. How else would you proceed?
In this case I think it's irrelevant because the stroke is cancelled, not because it's behind the tee.
