Grounds for disqualification

If you knew a 4 ball was already out, couldn't you have formed one yourself so as not to be held up? Or just play through them coz you're quicker... either would work for me
 
There wasn't a four ball down on the sheet though - the group had put themselves as 3 balls but then decided to change it up and play four balls instead.
In medal competitions - the club only allows 3 balls on the sheet. So there shouldn't be any four balls out on the course. They wouldn't allow me to play through either.

The question Fundy is raising though, it's easy to assume that the maximum number of players in each group is 3 because there are only 3 spaces on the sheet for each tee time, but do the club rules specifically state that 3 is the maximum number?
