Provided the card contains your handicap, the gross scores, and has been signed by you and your marker, it's valid.

It would, of course, be helpful to have your name, date, and name of the competition.

A pal of mine shot 22 points in a nine hole comp on Thursday, we entered his score on the computer and I noticed he had not put his name on his score card. He put it on but could he of been DQ if he had not put his name on. He had signed it.

Omitting your name is not a reason for DQ but it does make things difficult for your committee. Imagine the situation if everyone omitted their names.
