Playing from the teeing rea

wonder if you can help here, during a strokeplay competition one of our members struck his tee shot on the 4th tee and his ball landed in the teeing ground of the next hole(the 5th)
He then played his 2nd shot from the teeing area on the 5th.

Q: Should he have taken a drop, without penalty, to the nearest point of relief?

If he should have done the above but didn't, what should the penalty for this (if it is) transgression be?
 
someone once told me that when you play off a tee whether your tee or not, you should not replace a divot as it could give someone following an unstable lie?

I have this (with minor edits) from the R&A

"Such a Local Rule is not recommended but we have, on occasion, allowed it. It should only be considered if play from “other” tees is causing a serious problem. They are not necessarily meant to be pristine as with a Wrong Green.
"Teeing Area" and "tee" are defined terms and so it makes this a complex drafting issue. Difficulties may arise with clearly defining what comprises the "other" affected tees It should only be applied to specific areas not a blanket rule for all "other" tees
Substantial advantages or disadvantages arising as a result of players being compelled to take relief. Such as the relief area being on a severe slope or in the middle of surrounding gorse etc.

I think that is of more relevance to the green staff. They may miss replaced divots when they should be sand/seed filled properly.
