What’s this?

If you're measuring from A to B you'd count how many revolutions of the map wheel there were, then you'd run it across the scale, usually found at the bottom of the map the same number of revolutions.

If A to B was 10 full revolutions, and the map scale = 1 mile, and the wheel went across the scale 10 times, the distance from A to B = 10 miles.
