Pulling Left

Hi guys, some of you may recognise me, I haven't posted in a while but playing golf again now and need a little bit of advice.

My problem shot is a straight pull left, or a big hook left and I noticed this evening at the range that after the shot, if I lined my hands up to where the ball would be the club face was always pointing severely left.

I line up to the ball with my clubface pointing straight and my swing path isn't that hideous to create this angle....this pretty much means that the club is turning in my hand and destroying many rounds of otherwise happy golf.

Any solutions to this? My club grips aren't worn, my glove was a little used but I have the feeling the problem is deeper rooted....perhaps have a lesson focusing on my grip? Smaller grips?

It might not be your problem, but I have seen people subconsciously re-grip the club just before they start the backswing, and also re-grip at the top of the swing because the grip isn't secure.
 
I had this problem though it has now mostly gone - largely thanks to bob, I now mostly hit an acceptable draw.
My right hand was too far round so that it was practically under the shaft - couldn't help but roll my hands over. Even knowing that the habit was so ingrained that at times I still slip my right hand under almost as if it were part of the backswing.
