High hook off the tee

Hi, not sure where's this has come from however over the last two rounds there has been 6 occasions off the tee, where I've hit a high hook - nearly at a right angle. It's been bugging me as much be sometime win my set up at certain times as it' happens now and again. Can anyone diagnose the fault?

Is English your first language?

Is your ball starting off left then hooking?
And is your weight getting stuck on your back foot?

Good call Bob anyone would think you are a pro happens to me when I leave everything hanging back and have a high tee

iPhone doesn't allow me to communicate properly (I've written this twice!)

If 12 o clock was straight I'd say the ball starts 10:30 and ends at 9! Aka parrarel fairway!
