Yes and yes. Look on the St. Andrews trust website you will find all the t&c's there, the ballot closed around the end of august, but there are various other ways to play the old course depending on your budget

How does the ballot work? Is there a deadline each year and if so I take it I have missed it for this time?

Do you need to know who you are booking for ie the names in the 4 ball at the time of entering?

As AAC says check the trust website for details on when the advanced ballot will open and close next aug/sept for 2017. It also details how the remaining times for 2016 are allocated.
Also note there maybe some times become available in January, if anyone allocated a time and doesn't pay on time that slot it is then released.
You can change the 3 playing partners names after booking but not the lead name IIRC

Font forget winter golf offer just over £200 and you get a choice of all courses including the old, plus a £15 meal voucher too

we played mid April and it was stunning! No restrictions on using mats either :)

Dry your eyes mate and get up to Carnoustie. Now that's a course and wonderful people there too. No stupid ballots either!!! Book in and your done.

Even though we didn't get the Old (will still be trying in the 48hr ballot) we've just paid for the New and Jubilee which we did get and have booked our digs for the week.

Roll on July 2016!

You'll have a great trip hopefully the icing will be the Old in the 48hr ballot or daft 0 clock queue.
