Repairing a Deep Pitch Mark

What is the protocol for repairing a deep pitch mark that has 'dislodged' a piece of the green. Do you straighten out the piece and put it back as part of repairing the pitch mark - or just leave it out and 'compress' the surrounding surface turf to 'fill' what has been dislodged.
 
When it's quite a significant chunk that comes loose, I end up lifting a mound of roughly half a golf ball up and then sort of turn it in on its self before flattening with the bottom of the putter. Looks horrendous to start with but I find is the best way of making sure it leaves no indent in the green.
