what's a good pen make and type for ball marking that doesnt come off? ones I use are indelible but after a few hits they are coming away.
Also, anyone know of a place to get a template as everyone uses 1,2,3 dots, circles etc and with everyone using a prov1 I've played the wrong ball more than once as it had 2 red dots and a Titliest etc etc

Yep. Anywhere sells them. Your pro shop will sell the mini ones but you'll get a pack of proper size one from Smiths or somewhere for not very much money.

The black marker lasts all round in all conditions but the other colours are hit and miss.
I got a ball holder that allows you to put lines on the ball from pro shop.

Best way to mark your ball and give it a chance of lasting is to put dots in the depressions of the dimples, if you write on the raised sections of the ball, they will come off with contact of firm ground, sand, club faces etc. At least the dimples are recessed and will last longer and keep your ball looking tidier
