A club ran a mixed pairs competition where the better score of the 2 men per hole and the better score of the women per hole
counted. In effect the men pairs and the women pairs were playing a 4BB and the two scores were aggregated to achieve the final score.
Could/should this have been acceptable for handicap? As in reality the pairs were playing a 4BB over a measured course etc.
I’m not sure is there is a setting in IG for this format to be acceptable but scores over 42 points that meet all the rest of the criteria could be manually uploaded.
The winning 4 ball scores an ‘impressive’ 92 points and having, done the calculation, one of the players, if scaled up using the 4BB calculation, would have scored 48 points.

Were the men allowed to advise/assist the women (and vice versa)?
It would seem that they were playing as a team of 4

It's either...

A 4BBB aggregate competition, in which the pairs play independently according to 4BBB rules. This would be acceptable for handicapping. (IG setup would be a 4BBB comp with an overlay for team scores)

Or,

A 4-person modified best-ball competition, not 4BBB. This would not be acceptable for handicapping.

The Trevose May Festival is an example of the former (I believe). However, I suspect your example was the latter.

I’m afraid not

I believe you are right.

What are the differences between 4BBB aggregate competition and a A 4-person modified best-ball competition, not 4BBB i.e. how can you distinguish between the two?

There a few different rules applications, e.g. advice.

I'd say the most obvious indicator is whether the 4 players are in the same group or not. In modified best-ball, the four players would be playing together, whereas the pairs would usually be split up and play in different groups in an aggregate 4BBB.
