I am a doctor, although no expert on Primary Sclerosing Cholangitis, but a transplant is the definitive treatment. Results of transplant are generally good, although the patient will have to take anti-rejection drugs (probably) for the rest of their lives and there may be side effects and complications.
If your friend is being referred from outside the UK for a second opinion, presumably they will be referred to one of the leading units in the country, so that would also be a place where high numbers of transplants are carried out, and these days, it isn't quite as big a job as it used to be.
Also, the source of the donor is important, and a well matched liver, perhaps from a living donor, improves the outlook.
