Topping my driver

So one of my worst habits is topping the driver/3 wood of the tee.

Is there a simple way I can figure out what the cause is once I play the shot? Is it my right knee straightening? Weight transfer? Ball too far forward?

The thing is, I hardly ever top it at the driving range and have quite a strict practise in terms of the drives must be between two points. So I step up to the tee full of confidence but do have that nagging doubt! I do not step up with a clear head - plenty of swing thoughts.

I try and visualise and find a spot to aim at.

Any thoughts on how I take the confidence of the range to the tee?

When I do this its because I've been anxious to see the flight and looked up too soon.

On other occasions its because the I've let the ball creep back in my stance; try putting the ball well forward and then gradually move it back.
