Cannot find where it states what the Rules are for submission.
Specifically if there are temporary greens. Seem to recall more than 2. Also if there are temporary greens and the cup is wider than the normal size what difference does it make if any?

Supplemental cards require a measured course - the full rules for those being in the CONGU manual. If you can play Q comps on it you can submit a supplemental on it.

Large holes are definitely a no no.

You are correct in that not more than two temporary greens are permitted.
In addition the length of a course may not be reduced or increased by more than 100 yards (50 for 9 holes) without reference to the county.
All holes must be regulation size.
