Penalty Strokes

If someone takes their second stroke and plops this into the water or oob when they take their drop what number shot are they then playing?

If they've hit their second OOB they play their next from the position they played the last stroke from and add a penalty so playing 4. Same principle with a ball lost in a hazard but where you play the next shot from depends on the colour of the stakes defining the hazard but it'll still be the 4th shot
