I've done it again!

Gareth, I feel your pain. I did it twice on consecutive days of a 36 hole comp earlier in the season. Stood on the 16th tee +2 gross on the Saturday, +4 gross on the Sunday, before dropping a total of 15 shots over the last 3 holes to come in one under h/c and, to rub it in, lose the comp by 2 shots.

It's so easy to think "Ok, just need this drive to hit the fairway, then get it up around the green and I should make a 5 at worst" but that'll kill you. Just think about your swing, however you do that, and allow the result to be what it is. If you commit to the swing and the shot, you'll be fine. If you don't, you might get a bit quick, or you might look for the outcome of the shot before you've hit it and come up out of it too early. The result is unlikely to be what you want it to be. And, if you're thinking that way to begin with, you're likely to compound the first mistake with another one.

It really is as simple as taking one shot at a time. Easy to say, difficult to train yourself to do. You have to find a routine that works for you and work out how to silence any doubts and make yourself commit to every shot.
 
You really haven't blown a good score, you have probably played the last five 1 over your handicap.
Easy to do if it's a tough finish like it sounds from your SI (same at my club with 3 par 4s 430+ in the last five).

So you need to ask yourself at what point were you really aware of your score and and did it affect your approach to the final few holes?
 
I've found the best way to deal with this is to blow up on the front nine and then your over the moon coming in on or near your handicap.
