How is a hole measured

Stupid question but if a hole is say 250 yrds but its over a deep valley is it 250 as the ball flies....

or 250 as the ball roles

Thought it was measured up the middle of the fairway so if there is a dogleg it is measured to the middle of the fairway at the dogleg then to the centre of the green.
