You've got a decent platform to work with there, mate. The biggest issue is with your takeaway, which is far too far inside. Work on keeping the clubhead outside the hands as you take it back. It'll feel like it's going WAY outside, but it won't look like it on video.

I've recently solved this issue in my swing by focusing on the crease in my left wrist at address. All I do is work on maintaining that creae through the backswing. Like me, you're hooding the club back by losing that angle.

As others have said its a pretty good action.
I believe you problems come from the position at impact.
As you can see below , you've lost the good posture you had at address and straightened your back. When you couple this with not much leg drive or hip turn, that can cause problems.
My advice would be to try and maintain your spine angle throughout the swing and dont be scared to get ooff that back foot. This I believe will improve your ball striking and stop the inconsistency of your shots. :D

2 things I notice would certainly cause the bad shots that you said you hit

As someone as suggested you do take the club back a little on the inside and then at the top it looks to me like it is "stuck" behind your body
I would also assume (would probably be proved correct with front on swing) that you do not get your weight fully onto your right hand side at the top of the backswing.

If you approach the ball from the inside and your body is ahead of it then shots will either be lost to the right and hooked.

I would love to see a swing from the front.

The first thing I would get you to do is to try and fade/slice the ball (something I assume you struggle to do) this will change the movement and shpe of your swing and that way you would develop a better awareness of where the club and the weight in your swing goes.

Try some fades/slices on the range and let me know how you get on

Well in regards to blocking hooking etc there are a lot of causes...as this recent thread explains

I notice you have quite a dramatic lift at impact which generally will open the clubface (as it gets left behind).
Also your hands are quite low (near the knees at address) and you take the club back flat and inside the line..

therefore you also come into the ball quite flat which makes it hard to strike down onto the ball properly (gets a bit scoopy flicky)

Take a look at the picture above and see where your clubhead is...it's actually off the picture. Then take a look at this video of David Toms (he's actually hitting a 5,4 or 3 iron) yet at no point is his clubhead anywhere near as far hehind him. http://www.youtube.com/watch?v=XZuXp_qlNnw

If you get on plane you will find it a lot easier to stop blocking it, and shots hit out to the right have a greater tendancy to draw back in if you release the clubhead through properly. Be warned - if you raise your swing plane and come through straight you will probably start hooking it a lot to start with, until you get used to the change. Your wrists are currently doing a lot of unnecessary work and that's hard to get rid of in an instant.

Could this be exacerbated by practising with street shoes rather than wearing golf shoes, which would help give you a firmer platform to shift your weight from ?

Or are your golf shoes just damn fine street stylee ?
