Recurring swing faults?

Two of my swing faults are that my stance tends to get too narrow (feet too close together) and a tendency to take the club back too much inside the line on the backswing. Both faults emerged in Spades today, particularly as it was very windy and I was tending to lose my balance in the gusts.

What recurring swing faults do you keep chasing round in circles?

Standing a little closed, taking the club back inside then having to go out to in to get the club back to the ball. Straight pull or pull slice. Done it most of my golfing life

First one is easy to correct - stand with your feet further apart! The second one is also my fault. I find that the best way to correct it is to ensure that, at the start of the swing, I move the shoulders & nothing else until the hands & arms are forced to move by the momentum of the backswing.
 
I too can get too narrow with my stance with my 3 wood, causing me to lose balance and sway.
The other problem is NOT turning my hips towards the target on the downswing to start my swing, instead I merely turn them parrell on the railway track causing me to pull the ball.
 
Left arm collapsing on backswing. Whenever I stop using my tac-tic training aid it always creeps back in. Learnt my lesson this time - now an integral part of all practice sessions and my pre round warm up :)
 
Coming over the top of the ball. I've hundreds of ways of making it happen and can rustle up a low pull so easily it hurts. If I had a better swing and hours to practice I'm sure I could sort it but I'll have to live with it now and try to remember the right swing thoughts.
 
i generally have a nicely timed backswing but ,i find my self far too often swinging way to fast on the backswing ,the result is my tempo go`s and the shot isnt pured .
