I'm also a big fan of a "you're welcome" when someone says thanks, my 4 year old uses it.

Only time I can remember holding a door open to someone who didn't bother to say thank you was at Sunningdale. No not a forumer this year, but a couple of years ago to Hugh Grant.
I might have muttered knob as he walked away.
