We depend upon both our own knowledge of the rules and that of our playing partners to try and ensure that none of us break the rules during any round. Likewise it is incumbent upon all players in a group to point out any uncertainty arising during play around potential rule breaking, to proceed as the rules around the uncertainly demand; and agree to raise this on completion of the round. If no-one spots or suspects a rule break or a possible rule break during the round - then it might as well not have happened.
