What defines GUR and who can apply it?
I think the definition of GUR is hitting the actual putting surface and having a putt for birdie or less.
I apply mine myself - I always give myself a GUR if i'm in the fringe but I have a putt.
The same for fairways in reg as well - if i'm in the 2nd cut with a good lie i'd probably give it to myself.

Normally white or blue lines or blue stakes.
In theory applied by the committee, in practice the greenkeepers.
