Balls in motion

Playing in a comp the other day, I was just off the green and one of my PP's was just on. Both of us, without realising the other was also doing so, putted at the hole. The balls just missed each other.

Should either of us have been penalised?

What would be the correct procedure had the balls collided?

1. No
2. The ball played from the putting green is replayed, the stroke being cancelled. The ball played from off the green is played as it lies.(on the basis of both balls being in motion when they come into contact)
