I haven’t got ours to hand but EG have a very useful document that covers most of the bases and gives a template.

Thanks for the offer but I’ve received a comprehensive set and the club are going to move forwards with them as a basis for ours.
It did seem odd that there were none in our club and I’m happy they are going to introduce them.
