This is what really annoys me, front 9 today i shot 56 for a par 36 front 9 then go shoot 42 for a par 34 back 9. Ended up playing pretty much to my hcp but think i really should go to the range and hit a few balls and warm up propley first as it seems to take me 6 or 7 holes to warm up propley and feel like my swing is right.

On the plus side aswell my driving today was the best it has ever been, apart from the first where i hit a big hook the rest were on the fairway (got a lucky bounce off a tree on the 18th and it bounced back onto the fairway but it counts in my book haha)
 
Many golfers see this as a pattern. I used to see it more as sets of 6, my game comes together over the 1st 6, the 2nd 6 I start to score and the final 6 I have to ensure I do not fall foul of fatigue on the long rounds.

Really now I take it on a hole by hole basis at my home course, depending on the wind I know which holes to attack and which to hold back and get my nett par.

Warm ups are essential, as is regular practice, soon you will find only the 1st hole is a little rusty, I have scored 7 on the first hole at my course (par 4) in 5 competitions, won 1, 2nd once and 4th in another, all rounds under my handicap.
 
I always warm up before every round. If its a competition I'll go to the practice ground about an hour before hand (but make sure I don't hit too many shots or think about the swing), hit some chips, bunker shots and finish on the putting green about 15 minutes before my time. Even for a roll up with my mates I'll still hit some balls on the net and focus on tempo and contact.

I'm trying to play it one hole at a time as I don't see the point in a set of 6. You can't influence what has gone before or holes you haven't played. You can only play the next shot

If I started from cold, I dread to think how bad I'd be.

I always hit some balls. I personally like to use a net, since I don't get stressed about direction. (Sounds daft, but I tend to hit the ball straight enough for my needs!)

My best game this year was straight from the car to tee (via pro shop, I do pay sometimes! :))....BUT, I'd hit 200 the day before.
