Should the ball go back on the tee ?

When teeing off today I decided to give it everything I'd got.
Needless to say I made a complete cock up and smashed the driver into the ground.
I didn't touch the ball at all but it fell off the tee. Probably due to the ground shaking :D

Should I have played the ball from where it fell or place it back on the tee?
 
Declare unplayable and re-tee but then playing the 3rd shot.

If not declared unplayable, play as it lies
 
A follow-on question, if the ball fell off the teepeg and landed in front of it, and you decided to play it rather than declaring an unplayable and replacing it on the tee, could you remove the teepeg before playing your second shot?
 
All good, except that if you re-tee your ball, you are playing from where you played your previous stroke and are deemed to be playing under penalty of stroke and distance. See second statement of Rule 27-1a.

In simple terms you have made a stroke with the intention of playing the ball, but missed, same as an air shot, so it counts and the ball is now in play. Because it fell off the tee peg, you now have the options of playing your 2nd shot as it lies, or re-teeing and playing 3 off the tee.

Why would anyone take a penalty stroke to tee their ball up again when their ball is sitting on the tee? Large divot hole, perhaps?

If there was a large carry to the fairway that could only be cleared by taking driver? 'Driver off the deck' is not a shot in my armoury!

To be fair, it you are in this scenario, driver off the tee peg doesn't seem high up on the list of strengths either!

Best hope it's stableford - pick it up and go to the next tee.

Hashed a drive into deep, deep rough in a tie earlier in the season. Found my ball but had no chance of playing it so declared unplayable and went back to the tee. Opponent said I couldn't tee it up...
