Indicating tees played from on scorecard

Is indicating the tee played from in a competition mandatory and if so is the penalty disqualification?

I assume you mean to indicate it on the card. If so then the answer is no. Edit: Just re:read the post in conjunction with the title so yup you mean scorecard.

Date, name, handicap, the gross score for each hole and signed by marker(s) and player. That is all.

While agreeing that the rules do not require the player to indicate which tees were used, it would be sensible to do so if submitting a Supplementary.

True. Also necessary here in Ireland if you play better than handicap in a society away day so they can know which SSS to use.

A club may require the player to indicate the course he played.
Further, if he doesn't, the club cannot determine the SSS so cannot calculate any handicap adjustment.

Do you have a source reference for this? I could find nothing on any England Golf communication. Clearly it makes sense that the club has to know which tees were used.
 
Clause 21.6 A player intending to return a Supplementary Score is required to signify his intention prior tocommencement of play in the manner determined by the Affiliated Club.
